Before starting this application, please ensure you have your information, including your veterinarian's phone number, handy. You will not be able to save your responses and return to the application.

Please answer the following questions. We want to know a little about you and your family to ensure that the cat you requested is the best match for you. Remember that this is a lifetime commitment. We will ask you to sign a legal contract when adopting your new pet.

There is an adoption fee to partially reimburse us for spay/neuter and other care while awaiting adoption. The fees go directly to Love A Cat Enterprises, a charitable, non-profit organization, with IRS 501(c)(3) designation.

By my signature below, I certify that the above statements about myself, family, and other companion animals are all true. I understand that Love A Cat Enterprises has the right to disapprove this adoption application based on their adoption criteria when they feel it is not in the best interest of the animal offered for adoption.
